Provided an investigator submits his/her protocol in timely fashion, the
investigator may include the approval date on the face page of the application.
In the case of committed renewal applications, the investigator should use the
most recent date of approval on the application face page, even if the current
approval is from the prior year's submission. If no approval has been received
by the date of the deadline, the investigator should write "pending" on the face
page. Approval must then be obtained within 60 days, or the application will not
be reviewed by the sponsoring agency.
The Office of Sponsored Programs Administration is
responsible for providing certification of IACUC approval to funding agencies
when such certification was not included by the investigator on the application
face page.

The procedures for ensuring that discomfort, distress, pain, and injury

Investigators should also indicate that they have a made a thorough search
of the related literature and determined that there are no alternatives to
procedures which may cause the animals more than minimal or slight pain or
distress.
Investigators must describe the method of euthanasia to be employed. Again,
the information provided should match that appearing in the protocol.
