Indications for Ordering Genetic Tests
Genetic testing may be used to determine carrier status for individuals who have a family history of genetic disease or who belong to specific ethnic groups that are at increased risk for certain genetic diseases.
Genetic testing may also be performed to assist in the diagnosis of symptomatic individuals or to determine whether a fetus of a carrier couple is affected.

Who May Order Genetic Tests
By law, genetic tests may only be ordered by licensed Physicians or Laboratory Directors. Patients cannot self refer to a diagnostic laboratory for genetic testing. If you are a patient interested in genetic testing, please follow this link for information on how to obtain these services.
Steps in Ordering Genetic Tests
Step I: Review the list of tests and patients they are indicated for to select the tests you wish to offer to your patient.
Step II: Review the list of test prices and our payment policies and procedures. You must select a valid payment method for the tests to be done.
Step III: Print out and fill in our combined laboratory requisition and consent form available here. A separate form is required for each specimen submitted.
Step IV: Genetic testing requires an informed consent from the patient, or the patient's parent, or legal guardian. The information in our patient brochure may be used to help counsel the patient or guardian about the benefits, risks, and limitations of the testing you wish to order. If the patient is of Ashkenazi Jewish decent you may wish to have them view our video on genetic testing for Ashkenazi Jews. The patient should then read and sign the consent section of the requisition form.
Step V: Specimens must be collected, stored and shipped exactly as described in the Specimen Requirements document.
Step VI: Verify that appropriate Insurance information (if applicable) has been filled in and that the patient has signed the insurance section of the requisition form or included full payment with the specimen.
