Medical
Eligible employees* may choose from one of five options:
- UnitedHealthcare PPO Basic and Plus options: UnitedHealthcare PPO Basic and Plus plans allow the participant to have flexibility when receiving care, for there are no Primary Care Physicians required and referrals are not needed to see specialists. Plan participants have the option to receive care from NYU Medical Center's "Top Tier" health providers, which results in the least out-of-pocket expense.
- HMO Options: Through an HMO, the participant receives In-Network care from doctors, hospitals, and other providers. Out-of-network services are NOT available and a primary care physician must be selected to coordinate the health care needs of the plan participant, including referrals to specialists. Employees have a choice of HMO plans by Aetna, Empire BlueChoice, and HIP.
- Decline Medical Coverage: If you have medical coverage through another plan (such as through your spouse's employer), you may decline medical coverage. When declining medical coverage, prescription coverage is also declined. Because we believe that it is important for all eligible employees to have medical coverage, we require proof of other coverage when choosing this option.

Prescription Drugs
Eligible employees are offered prescription drug benefits administered by Express Scripts. To have prescription drug coverage, employees must be enrolled in one of the medical plans offered by NYU Medical Center.
Dental Plans
Eligible employees* may choose from one of three options:
- Met Life Dental PPO Basic and Plus Options: Participants in MetLife Dental PPO (basic or plus options) can receive dental care in- or out-of-network. Both Dental PPO options cover the same services, although at different levels. Your out-of-pocket costs are generally lower when you use an "in-network" dentist.
- CIGNA DHMO: Participants in CIGNA DHMO must select a Primary Care Dentist and receive care from in-network providers only. Out-of-network services are not covered under this plan.
- Decline Dental Coverage: Proof of other insurance is not required when declining dental coverage.
* Dental plan options vary for certain employee groups, who may also be required to satisfy waiting periods before becoming eligible for dental benefits.
Vision Plan
Eligible employees may elect the vision plan administered by Eye Care Plan of America (ECPA). This plan helps participants pay for eye examinations, eyeglass frames and lenses, or contact lenses. Participants have a choice of accessing care through in- or out of network providers, although richer benefits are received when obtaining services from a participating ECPA provider.
Life Insurance
All employees are required to have a minimum amount of term life insurance offered by Aetna. Eligible employees may also purchase additional amounts of life insurance for themselves and also for their eligible dependents.
Accidental Death and Dismemberment (AD&D)
Eligible employees may purchase AD&D benefits equal to the amount of life insurance elected. AD&D coverage provides a benefit if you die or become dismembered as a result of an accident. If the accident results in your death, these benefits are paid in addition to your life insurance coverage. AD&D benefits are not available for dependents.
Health Care & Dependent Care Reimbursement Accounts (HCRA/DCRA)
HCRA & DCRA (also known as "Flexible Spending Accounts") provide eligible employees with a way to pay for certain health care and dependent care expenses on a pre-tax basis, provided these expenses are not reimbursable under any health insurance program. Contributions to HCRA & DCRA exempt from federal and state (where applicable) income taxes, as well as social security and Medicare taxes.

Disability
Disability benefits protect you and your family from loss of income in the event that you are disabled as a result of illness or a non-occupational injury. Most employees are eligible to receive short-term disability benefits up to the initial 26-week period, while only employees in eligible job classifications may receive benefits for permanent or long-term disability periods beyond the initial 26-week period. All disabilities must be certified by the insurance company (UNUMProvident) before benefits are paid.
