The BLR publishes two 160-page issues per year. Each issue includes, on average, eight works of fiction, four essays, and 20 poems, and features a historical photo from the Bellevue Hospital archives on the cover. The journal has published works by a great variety of authors, from medical students to Pulitzer Prize-winning writers. Submissions to the BLR have risen steadily since 2000. As of January 2005, more than 7,000 works of literature have been submitted for review.
Readership has also risen steadily. The BLR's current print run is 4,000 copies per issue. Its paid circulation in 2004 was over 1,300 (an increase of almost 100 percent over the prior year); another 1,000 copies are distributed to bookstores throughout North America. The remaining copies are sold individually through the website and at events, given as complimentary copies, or donated to educational programs.
