Case Management and Care Coordination

Case Management and Care CoordinationConsortium case managers and clients work together to identify and address health and social issues that affect the client’s ability to access resources and services needed to improve and maintain their quality of life. Based on the individual needs of each client or family, case managers work with the client to develop interventions that help empower the client and promote the highest level of independent functioning possible for the client and family.

Case managers frequently work with clients in the following areas:

  • Coordination of care across multiple providers
  • Housing placement assistance
  • Assistance with basic needs
  • Escorts with appointments
  • Access to care
  • Risk Reduction
  • Family services
  • Home visits
  • Crisis intervention
  • Mental health support
  • Substance use support
  • Legal support

In order to help families juggle the many providers they and family members use, the Consortium has developed several strategies to facilitate referrals between hospital social workers and community case managers at participating agencies. These include:

  • spearheading monthly Consortium Case Management meetings with social work/case managers from funded Consortium sites that provide opportunities for peer support and case conferencing
  • providing funded social work/case managers with clinical consultation to forestall burnout and address worker concerns
  • facilitating on-site placement of community-based case managers in clinic settings to streamline referrals and increase case management collaboration between the community and clinic
  • developing strategies to ensure that quality case management and social services are provided
