ServicesThe Lower New York Consortium's goal is to improve access to and maintenance in care for HIV-infected women, children, youth, and their families by providing family-centered, case managed HIV/AIDS primary and specialty care linked to community case management.

Family-Centered Care
It doesn’t matter who in the family is infected with HIV—everyone is affected. Family centered care providers recognize the importance of the family in the lives of clients. Programs are designed to meet the special needs of families.

Services for Women
Specialized services cater to pregnant women, those at high risk and women of color.

Project YOUTH
Serving children from ages 13 to 24, Youth Outreach United for Teen Health aims to reduce risk levels through education and to increase access to services.

Case Management and Care Coordination
Case managers and clients work together to identify and address health and social issues that affect the client’s ability to access resources and services needed to improve and maintain their quality of life. Another Consortium goal is to increase case management collaboration between the community and clinic.

Our various affiliations make clinical trail referrals more accessible to our patients.
